Please log into the faculty survey at https://secure.las.illinois.edu/CVStorage using your Active Directory (AD) username and password (this link will bring up a new window).

You will first come across an Initial Setup Wizard. Review the Terms and Conditions and click "Next". This will take you to your User Profile. The User Profile has four screens or pages, "User Information", User Publications”, “Department Relationships, "Proxy Permissions", and "Finished". Click on the "Next" buttons at the bottom of the page to navigate, or use the links to these screens on top of the page, right below the heading “User Profile”.

• User Information: The fields on this page have been pre-populated with your information from the campus directory. You do not need to change anything unless you see glaring errors.
• User Publications: Please enter your five most recent publications relevant to international or area studies.
• Department Relationships: Please ignore this screen.
• Proxy Permissions: Please complete this screen ONLY if you wish to delegate the updating and correcting of your data to someone else, i.e. your assistant.
• Finished: Click on the "Finish" button to finalize the initial setup of your User Profile.

After completing the User Profile, please click on the 'Title VI' tab on top of the page to navigate to the Faculty Survey page. The questions asked in this survey should be self-explanatory. They are all related to the information we need to include in our Title VI grant applications.

When finished with the Faculty Survey, do not forget to click on “Save Survey” before you sign out.
